The 2004 survey collected data from approximately 30,000 individuals ages 12 and older from either a questionnaire dropped off by the interviewer and collected at a later time or through a computer-assisted telephone interview (CATI). Data were collected on the use and effects of tobacco, alcohol, pharmaceutical drugs, and illegal drugs.
