The sample sizes were 26,685 participants in the urban region, 2,797 participants in the mountains region, and 1,954 participants in the islands region. The NHIS 2001 included five questionnaires: one for the household, one for participants over the age of 12, one for 12 year olds, a self-administered questionnaire for 12 to 19 year olds, and a self-administered questionnaire for participants ages 20 to 65. Questionnaires collected sociodemographic information, health status (including SF-36), health care use, and health behaviors including drug use and sexual behaviors.
