Two questionnaires were used, one for households and one for school and health facility service providers. The survey also covered various aspects of service provision including availability, accessibility, utilization, satisfaction, limiting factors, and constraints. A multi-stage cluster sampling was used. A total of 10,200 households were included in the sample with a response rate of 95.5%. Unlike the 2004 survey, estimates could not be done at district level. Instead, estimates are provided for 17 district groupings or subregions.
