Individuals of interest were women ages 12-49 and residents in the sampled districts. A questionnaire was used for all pregnancies ending beyond 22 weeks and for all maternal and perinatal deaths. Maternal and neonatal verbal autopsy forms were also used. The survey covered in-facility and community births as well as maternal and perinatal deaths.
